How to add, edit, and customize interactive forms on your website pages

Form elements allow you to make it easy for your users to submit information to your district through the Submissions tab on your Streamline site and/or via email to your staff. You can customize forms to include a number of different field types, such as dropdown menus, radio buttons, and even file uploads.

Add a form to your page

  1. Navigate to the page and click Edit this page at the top right of the screen:  Screen Shot 2023-03-28 at 10.00.13 AM
  2. Below the Title, in in the Main Body Content area, locate the + icon  to the left of the location where you want to add a file, and click the icon to bring up the Insert Element menu (if you don't see these icons, simply click to the left, outside of the Main Body Content area, to make them appear)
  3. Choose the Form element and click Insert in the top-right corner:
  4. For each field you want to add to your form:
    1. Add a prompt under Label/question
    2. Choose a field type under Type
    3. Indicate if the field is required under Req'd
    4. For additional fields, click the Add field button:
  5. Click Save when finished:

    Set up email assignments & notifications for a form

    1. While editing the page where your form resides, hover your mouse anywhere over the form and click your left mouse button 
    2. Click on the Form settings tab:
    3. From the dropdown under the Assign new form submissions section, choose the user you want to assign this form to. All new submissions to this form will be assigned to them and they will receive an email alert when a submission comes in (learn more about managing users here)
    4. Optional: You can add additional emails to be included on notifications for this form by typing in each email address, separated by a comma. They will not be included on assignments. Example:
    5. Click Save when finished:

    Add fields to a form

    1. While editing a page, hover your mouse anywhere over the form and click your left mouse button 
    2. Click the Add field button:
    3. Add a prompt under Label/question
    4. Choose a field type under Type
    5. Indicate if the field is required under Req'd 
    6. Click Save when finished:

    Remove fields from a form

    1. While editing a page, hover your mouse anywhere over the form and click your left mouse button 
    2. Locate the red trashcan icon next to the field you want to remove and click on it:
    3. Confirm you wish to remove the item by clicking the green Remove button:
    4. Click Save when finished:
